You are being invited to take part in a research study conducted by Sarah Stevens of Palm Beach Atlantic University. Before you decide to participate in this study, you should read this form and ask questions about anything that you do not understand.
Description of Project
The purpose of the study is evaluate the effectiveness of the Life Alliance Organ Recovery Agency organ procurement opportunity, as well as Lunch and Learn sessions.
Explanation of Procedures
You will be asked to fill out an online survey. As part of the survey, you’ll be asked if you would be willing to participate in a phone and/or live interview.
Online survey: less than 5 minutes
Optional Interview: 15 to 20 minutes
Risks or Discomforts
No direct benefits to participant. However, data collected will be used to continue to improve Palm Beach Atlantic University School of Nursing curriculum and experiences.
The results of this participation will be anonymous unless participant chooses to participate in interview. Information collected via interview will remain confidential and be de-identified in research reporting.
Inclusion Criteria for Participation
- You must be 18 years of age or older to participate in this study.
Use of Online Survey
IP addresses will not be collected.
